Extra Curricular Sports Physicals for 2013-2014 School Year
According to the Kansas State High School Activities Association (KSHSAA), every student who participates in a school sponsored sport, including cheerleading, must have a current physical on file with the school. That physical must be taken on or after May 1st to be accepted for the upcoming school year. On MAY 1st, USD 362 will be offering the opportunity for Prairie View students to get this physical completed at school. Jaccard Chiropractic will be here during the day giving physicals to students who have registered through the office prior to May 1st. The cost of this physical will be $15.00. Checks can be made payable to Jaccard Chiropractic. You may pick up a physical form in your child’s respective office, or print it off from the KSHSAA website. There is also a link to this form on the front page of the school’s website.
Any student wishing to take their physical here at school must turn into the high school or middle school office by Tuesday, April 30th the following:
1) Their Completed Physical Form
2) $15.00 payment attached to the physical form
THE PHYSICAL FORM.
You may pick up the KSHSAA Physical Evaluation Form (PPE) at your student’s respective school office, the KSHSAA website or the school’s website. This form needs to be completed by the student’s parent/guardian. This completed form needs to be returned to the HS or MS office with payment attached by Tuesday, April 30th. To complete this physical form, please fill out the following:
Page 1 - Fill In All Information
- Make sure on the Grade you indicate NEXT year’s grade level.
Page 2 - At the top:
- Name
- Date of Birth
- Date of recent immunizations
Page 3 - Nothing
Page 4 - Student’s Printed Name
- Questions 1-4
- Parent and Student Signature
