Physical For 2011-2012 School Year Athletics/Cheerleading
If you are planning on playing sports or cheerleading for the 2011-12 school year, you will need a new physical on file with the office. If you took advantage of the school offered physical in May then your physical for the 2011-2012 school year is on file with your respective school and you do not need to do anything else.
If you did not choose to take advantage of the school offered physicals, then prior to the first practice date of the sport you are choosing to participate in or cheerleading, you will need a signed PPE form from a doctor. The following link will allow you to download the appropriate form, or you can pick one up in the high school office until June 11.
